PROBLEM:
If I make a file structure change (add a field/ delete a field or change a size of a field) all other fields adjust the offset correctly except for overlap fileds.  Why is that?

ANSWER:
Overlap fields are logical fields (they take no physical space on the hard drive).  All other fields are physical fields (that actually have a size).

Whenever you alter a file that contains an overlap field, you must determine if the overlap will be affected and correct it manually.  You also may have to check the indexes in case the index is referring to the overlap instead of the actual fields part of the overlap.
